How to format a title page for a script

I am writing a script for a short film, and I am not sure what goes on the title page. I have the title, the author's name, and the date. Is there anything else that should go on the title page?

The title page of your script should include the following:

-The title of your script in all caps
-The type of script it is (e.g. feature, short, TV show, etc.)
-Your name
-Your contact information (e.g. email, phone number, mailing address)
-The date
-An optional copyright notice
