A screenplay title page is a single page at the beginning of a screenplay that contains the title of the work, the name of the author, and other relevant information.
There are a few elements that must be included on a screenplay title page:
-The title of the screenplay in all caps. Centered on the page.
-The name of the author. Centered on the page. If the screenplay is co-written, both authors' names should be listed.
-The contact information for the author. This should include the author's mailing address, phone number, and email address.
-The WGA registration number. This is a number assigned by the Writers Guild of America. It is not required, but it is recommended.
-The date. The date should be the date that the screenplay was completed.
